[Af_List] cd duplication and printing

If you don't end up taking up David's generous offer, my wife did the
Salad Bones CD through Amstore, packaged in cardboard sleeves, and was
very satisfied with the result.

They treated her pretty because they liked the idea of the CD, and seem
generally to like creative people.
